Your completed Application for a Permanent Resident Card (IMM 5444).
Your fee payment receipt (IMM 5401), filled in and stamped by a Canadian financial institution.
Your completed Supplementary Identification Form (IMM 5455), with the photo correctly
attached and your signature in black ink inside the white box of the Client Signature area.
Your current PR Card, if you are applying for a renewal or to replace a damaged card.
A certified copy of
• your current passport or the passport you held at the time you became a
permanent resident; or
• your certificate of identity issued by the Minister of Foreign affairs; or
• your refugee travel papers issued by the Minister of Foreign affairs.
Note: The certified copy you submit should show only the document number, your name, photo-graph
and date of birth.
